Purpose There are numerous causes of left ventricular hypertrophy (LVH), ranging from physiological adaptation to athletic training or increased afterload to more severe pathological hypertrophy, as seen in hypertrophic cardiomyopathy. Differentiating pathological hypertrophy from physiological hypertrophy can be challenging, but it is crucial for determining the prognosis and treatment. Cardiac magnetic resonance (CMR) strain imaging, using both 2D and 3D modalities, is an emerging non-invasive tool for myocardial tissue characterization. This study aims to evaluate the effectiveness of CMR-derived strain parameters...
Read more Methods and materials Data from 124 participants were analyzed in the study, including 79 patients with HCM, 21 with LVH due to hypertension, and 22 healthy controls (CT). CMR imaging was performed using a 1.5T Vantage ExelArt scanner (Toshiba), with cine imaging in short-axis, two-chamber and four-chamber long-axis views. Strain analysis included longitudinal, circumferential, and radial strain measurements in both 2D and 3D formats. Post-processing was performed using commercially available specialized software. Standard statistical methods were employed to analyze the data, with a...
Read more Results In this study, healthy controls, with an average age of 47.5±15.2 years, were younger than both the HCM patients, who averaged 53.9±10.5 years, and the LVH patients, who averaged 58.5±14.5 years (p=0.01). There were no significant differences in sex distribution between the groups. Several strain parameters exhibited significant differences between the HCM and LVH groups, pointing to distinct myocardial deformation patterns [fig 1] . Specifically, myocardial strain analysis revealed that HCM patients exhibited significantly reduced radial contractility. This was evidenced by lower...
Read more Conclusion Overall, CMR-derived 2D and 3D strain parameters, particularly radial and longitudinal strain, can effectively differentiate between hypertrophic cardiomyopathy and hypertensive left ventricular hypertrophy. These results suggest that CMR strain imaging could play a crucial role in clinical decision-making. Further research is necessary to explore the full potential of strain imaging in enhancing diagnostic accuracy and improving patient management.
