Purpose In Dynamic Contrast Magnetic Resonance Lymphangiography (DCMRL), lymph vessels are selectively contrasted and displayed in their anatomical context. This allows visualization of their anatomy and changes in various diseases, as well as changes in the flow of lymph. These changes can lead to chylothorax, pericardial effusion, chyluria and protein-losing enteropathy. The present study explored the potential of DCMRL to significantly influence subsequent treatment decisions.
Read more Methods and materials Patients:The course of 10 consecutive patients aged 3 months to 24 years who underwent lymphangiography for different indications is examined. The diseases include chylothorax, chylopericardium, plastic bronchitis, protein-losing enteropathy, chyluria.Procedure:A 25-gauge needle is inserted into the central portion of an inguinal lymph node on each side under US guidance. Gadolinium contrast agent is applied to the lymph nodes and contrast flow is visualized on MRI using TWIST and FLASH sequences with dynamic temporal and spatial resolution.
Read more Results Patients with hyplastic left heart syndrome: 5 patients aged between 3 months and 15 years with hypoplastic left heart syndrome and chylothorax, in 3 patients also with protein-losing enteropathy. DCMRL demonstrated patency of the thoracic duct in all patients. In 3 patients, DCMRL showed that the contrasted lymph from the lower half of the body flows retrogradely into the thickened duodenal wall and from there passes into the intestinal lumen (protein-losing enteropathy). Since patency of the thoracic duct could be demonstrated,...
