Learning objectives Increased unilateral hemithoracic transradiancy on chest X-ray has a range of important differential diagnoses. Utilizing a series of cases, we propose an approach to review these potential diagnoses and highlight key learning points to help narrow the differential.

Accurate identification of the underlying cause requires careful evaluation by radiologists to ensure appropriate diagnosis and subsequent management. We have created an approach to the evaluation of a transradiant hemithorax through the use of a flowchart (Fig 10)
