Purpose Radial sclerosing lesions (RSLs) are benign breast lesions defined by fibroelastosis, which causes entrapment of epithelial elements within the stroma (Figure 1) [3] [fig 1] . These include radial scars and complex sclerosing lesions (CSLs), differentiated mainly by their architecture and sometimes by size [1]. Both types are idiopathic and not linked to prior trauma or surgery; their etiology may involve localized inflammation or chronic ischemia leading to slow infarction [2].Radiologically, radial scars feature a central fibroelastic core with ducts radiating outward,...
Read more Methods and materials Retrospective, descriptive study. We reviewed our database of Core biopsies (CBs) performed under US guidance (14G and 8G) from 2018 to 2024. Cases diagnosed with CSL were selected. Imaging, and anatomopathological data of the CBs and surgeries were analyzed.
Read more Results We evaluated 44 lesions (median size: 20 mm, range: 15-40 mm) in 41 patients (median age: 49 years, range: 32-82) (Table 1) [fig 1] . All cases exhibited architectural distortion on mammography (Mx) and tomosynthesis, along with “Cat´s Whiskers” Sign on US.The “Cat´s Whiskers” Sign manifested itself as a composite image of a central nidus and long spicules (more frequently) converging towards it from both sides, which are only visible in a single plane because the lesion is flat (Figure 3-4-5) [4] [fig 3] [fig 4] [fig 5] . Compared...
Read more Conclusion The prevalence of radial sclerosing lesions (RSLs) has increased with the wider adoption of screening digital breast tomosynthesis (DBT), which facilitates the identification of more architectural distortions. The "Cat's Whiskers" sign plays a crucial role in enhancing the identification of RSLs during targeted ultrasound assessments, thereby streamlining the diagnostic process through ultrasound-guided core biopsy and wire localization prior to surgical intervention. In our study, ultrasound did not effectively differentiate between pure RSLs, which accounted for 64%, and those associated with premalignant or...
