LCS through LDCT has been implementing worldwide. However, pulmonary and extra-pulmonary incidental findings unrelated to lung cancer, ranging from benign abnormalities to clinically significant conditions, are frequently identified. These findings often pose management challenges for referring physicians, while increasing patient anxiety and healthcare costs. Guidelines such as the 2023 ERS/ESTS/ESTRO/ESR/ESTI/EFOMP statement (1), the ACR quick reference guide (2) and the NHS quality-assurance-standards (3) emphasize the importance of balanced reporting to avoid unnecessary investigations while ensuring clinically relevant findings are addressed.
