Luxury perfusion is a vascular response to ischemia that is characterized by increased perfusion to the infarcted area in order to meet the local metabolic requirements, through dilatation of multiple vascular channels, particularly venous channels.
This is observed in the infarcted area of the brain 24-48 hours after ictus, in both treated and untreated cases, usually in the subacute phase, but has been documented as early as within the first 24 hours. The peak occurs in the second and third week, gradually fades over the following weeks, and can last up to a year. It is considered to occur as a result of the blood-brain barrier breakdown, neovascularization, and dysfunctional autoregulation.
