Purpose Prostate cancer is the second most common malignancy in men worldwide, emphasizing the need for accurate, efficient, and accessible diagnostic methods. While multiparametric magnetic resonance imaging (mpMRI) is currently the gold standard for prostate cancer detection and risk stratification, it requires contrast administration, making it more costly and time-intensive.Biparametric MRI (bpMRI), which excludes dynamic contrast-enhanced imaging, has emerged as a potential alternative, offering advantages such as shorter acquisition times and lower costs while still providing valuable diagnostic information. This study...
Read more Methods and materials Romania, between January 2020 and November 2023. All cases included T2-weighted imaging and diffusion-weighted imaging (DWI) for bpMRI, whereas mpMRI additionally incorporated dynamic contrast-enhanced (DCE) sequences.Prostate volume was measured on both imaging modalities, and lesions were assigned PI-RADS scores based on the Prostate Imaging Reporting and Data System (PI-RADS) guidelines. To assess the reliability of measurements and PI-RADS scoring, inter-rater agreement was evaluated using intraclass correlation coefficients (ICC) and kappa statistics. The study also compared the frequency of different PI-RADS categories...
Read more Results The analysis revealed an excellent agreement between bpMRI and mpMRI for prostate volume measurement, with an ICC value of 0.9963 for single measures, indicating near-perfect reliability. This finding suggests that bpMRI provides highly consistent and reproducible prostate volume assessments, comparable to mpMRI.  [fig 2] For PI-RADS scoring, substantial agreement was observed between bpMRI and mpMRI, with an ICC value of 0.9070. However, a slightly higher degree of variability was noted in PI-RADS assignments using bpMRI compared to mpMRI.  [fig 3] In particular, mpMRI was more likely...
Read more Conclusion Despite its limitations, bpMRI offers several distinct advantages over mpMRI, including reduced costs, shorter scan times, and the avoidance of contrast agents, which can be beneficial for patients with renal impairment or contrast allergies. These factors make bpMRI an attractive option for routine prostate cancer screening, particularly in resource-limited settings where access to contrast-enhanced imaging may be constrained. However, mpMRI remains the preferred modality for comprehensive prostate cancer evaluation, especially in high-risk cases where accurate lesion characterization and staging are critical.Future...
