A thorough understanding of subtle radiologic signs, combined with clinical history, is essential for accurate differential diagnosis and optimal patient management. Through a series of unique cases—including incidental benign lesions like splenic cysts and hemangiomas, vascular pathologies such as splenic infarcts, and infectious-related lesions like cat scratch disease—we provide valuable clinical insights into the spectrum of benign splenic conditions. Recognizing key imaging features in the clinical context enhances diagnostic accuracy and underscores the vital role of imaging in splenic lesion evaluation.
