Initially 8 of the 9 patients presented epileptic symptoms. Patient number VIII had a headache as the main symptom, which lasted for 2 weeks and was the main cause of the MR examination - in this case the lesion was located in the left thalamus.
In our study 3 of the 9 tumors were situated in the temporal lobes. Other tumors were located extratemporal: two in the parietal lobes, one in the occipital lobe, one in the frontal lobe, one in the left thalamus and one in the basal nuclei between the left lateral ventricle and the left insula.
In 8 cases tumors appeared as a solid-cystic mass. In the case of patient number VI the tumor appeared as a solid mass, which looked similar to cortical dysplasia, but with slight contrast enhancement.
The mass effect was observed in patients VI and IX. Oedema was observed in patients VII and IX. Contrast enhancement was observed in 6 of the 9 cases. The "salt and pepper” sign in T2WI, as one of the specific characteristics of PLNTY was observed in 8 of the 9 cases.
In 3 cases (IV, V, IX) a brain MR examination was performed at different time intervals.
In two cases (IV, V) the interval was not planned- these two patients left from medical observation, but returned after 2 and 6 years respectively. Over time, in subsequent MR scans, morphology changes of the lesions were observed. Initially the lesions had cortical dysplasia MR features, after 2 and 6 years respectively MRI features were typical for PLNTY.
In the third case (IX) the patient was under observation for 5 years in our Institute and brain MRI was performed once a year, before a decision of surgery treatment was made. Over the 5 years the lesion increased slightly and the contrast enhancement appeared.
In the immunohistochemic (IHC) results all cases exhibited intense labeling of CD34, Olig2 and partial labeling of GFAP. One case partially presented p53. Five cases had no presence of p53. Synaptophisin was partly presented in five cases. NF exhibited in three patients. All cases presented low level of Ki67. In the genetic results, the BRAFV600E fusion was not found in any of the cases.
